Welcome, the following links are to some forms I made up using HTML. The intent is to provide some easy forms to print for use in the theatre. I tried to make them nice to use, easy to read, and quick to load. Most of the forms I copped from the Backstage Forms book by Broadway Press. NB: Turn off the Header and Footer sections in Page Setup. Otherwise, the forms will not fit perfectly. Check in Print Preview before printing. The margins should be .50" all the way around.
